June 23, 2018 in Middletown.  He was born in Hamilton, OH. on Dec. 7, 1961 to Freddy and Helen (Prater) Keith.  David is survived by his wife of 20 years, Phyllis Keith., 3 children, April (Adam) Neeley, Trenton, OH., David (Misty) Keith Jr., Lenoir, NC., Andrew (Stephanie Abner) Beltz, Hamilton, OH., 11 grandchildren, Damon (Shelby), Alex a.k.a Fred, Madison, Emily, Kane, Elijah, Tanner, Caden, Zella, Madelynn, Mya and 7 siblings, Fred (Janis) Keith Jr. Eaton, OH., Joyce (Thomas) Banks, Trenton, Charlie (Brenda) Keith,  Bill (Gail) Keith, Shirley (David Norvell) Keith, all of Middletown and Priscilla (David) Banks, Trenton, OH., mother-in-law, Alice Jewell, Beattyville, KY., several brothers-in-law and sisters-in-law, numerous nieces and nephews who all loved him very much. Also survived by his dog, BooBoo. He was preceded in death by his parents.  Funeral services will be held 12:30 P.M.
